From Tanzania to the Region: Sharing the ADDO Experience
For more than a decade, Apotheker has been at the forefront of supporting the development, implementation, and promotion of the Accredited Drug Dispensing Outlet (ADDO) model—an innovative public-private partnership approach that has transformed access to quality pharmaceutical services at the community level.

Working in collaboration with partners such as Management Sciences for Health (MSH), Apotheker has provided technical expertise to governments, regulatory authorities, and development organizations seeking to strengthen community-based pharmaceutical services through the adoption of ADDO-like models.

In Tanzania, Apotheker supported MSH, the Ministry of Health, the Tanzania Medicines and Medical Devices Authority (TMDA), and the Pharmacy Council in hosting learning and exchange visits for delegations from Uganda, Malawi, Zambia, Madagascar, Liberia, Nigeria, Bangladesh, Ghana, Burundi, and Sierra Leone. These visits provided an opportunity for countries to learn from Tanzania’s experience in designing, regulating, and scaling the ADDO model.

Building on this experience, Apotheker has also worked directly with regulatory authorities in countries including Liberia, Zambia, Burundi, Bangladesh, and Nigeria to develop the policies, guidelines, training materials, and implementation tools required to establish and operationalize ADDO-like models within their own health systems.

Through these efforts, Apotheker continues to contribute to strengthening pharmaceutical service delivery, expanding access to quality medicines, and supporting sustainable health system innovations across Africa and beyond.

The following projects represent Apotheker’s documented consultancy experience in pharmaceutical services, medicines access, ADDO systems, regulatory strengthening, training, digital tools, evaluations, and health-system assessments.

Support to Infusion Units Program Evaluation

Apotheker supported the evaluation of the Support to Infusion Units Program for St. Luke Foundation and Kilimanjaro School of Pharmacy. The assignment reflects Apotheker’s role in reviewing pharmaceutical service initiatives and generating practical learning for institutional improvement.

Online Pharmaceutical Marketplace Feasibility Study

Apotheker conducted a feasibility assessment for an online marketplace platform intended to improve access to medicines and health commodities in Tanzania. The work explored how digital market systems could support medicine availability, private-sector participation, and improved access for end users.

ADDO Learning Visits and Technical Support

Apotheker coordinated learning visits from African countries to Tanzania’s ADDO program and provided technical support to countries interested in adopting the model. The assignment positioned Apotheker as a practical knowledge partner in community pharmacy accreditation and medicines access systems.

Private-Sector Supply System Assessment for GPO Development

Apotheker conducted a rapid assessment of the private-sector supply system to inform the establishment of a Group Purchasing Organization. The work contributed to practical thinking around pooled procurement, medicine supply efficiency, and stronger private-sector supply channels.

Health Shops Program Documentation in Zambia

Apotheker documented the progress of implementing the Health Shops Program in Zambia. The assignment supported project learning, implementation review, and the capture of lessons from regional medicine access models.

Paper Analytical Device Research for Medicine Quality Testing

Apotheker supported operational research on the use of Paper Analytical Devices by TFDA inspectors to detect fake medicines in retail outlets. The work connected field-level inspection with innovative medicine quality testing tools.

PointMed Pilot for ADDO Business Support

Apotheker coordinated a pilot introducing PointMed, a point-of-sale and patient record software for Accredited Drug Dispensing Outlets in Mkuranga District. The project demonstrated how digital tools can strengthen private medicine outlet operations, records, and service delivery.

Resource Mobilization Strategy for Accredited Medicine Stores in Liberia

Apotheker developed a resource mobilization strategy to support the scale-up of Liberia’s Accredited Medicine Stores Program. The assignment combined strategic planning, financing considerations, and medicine access scale-up thinking.

Bangladesh Retail Drug Outlet Regulations and Inspection Guide

Apotheker supported the Directorate General Drug Administration and Pharmacy Board of Bangladesh to develop regulations, standards, and an inspection guide for retail drug outlets under the Bangladesh Pharmacy Model Initiative.

Appointment System for Option B+ HIV Services in Mbeya

Apotheker supported the introduction of an appointment system in health facilities integrating Option B+ HIV services in Mbeya Region. The work strengthened service organization, patient flow, and continuity of care within facility-based systems.

Pharmaceutical Management Curriculum Update

Apotheker supported Muhimbili University School of Pharmacy in updating the curriculum for pharmaceutical management training. The assignment strengthened pharmacy education content and contributed to professional capacity building in pharmaceutical systems management.

Holistic Assessment of ADDO-Served Communities

Apotheker conducted a holistic assessment of healthcare-seeking behavior, medicine availability, medicine use, and stakeholder perceptions in communities served by ADDOs in Tanzania. The work generated evidence for improving community-level medicine access and service delivery.

ADDO Owner and Dispenser Motivation Study

Apotheker explored the motivation of ADDO owners and dispensers and how accreditation influenced dispensing behavior, medicine use, antimicrobial resistance knowledge, and treatment guideline awareness. The study supported better understanding of pharmacy practice behavior at outlet level.

Liberia Inspector Training Manual and Inspection Strategy

Apotheker supported Liberia’s National Medicines and Health Products Regulatory Authority to develop an inspector training manual and inspection strategy for Accredited Medical Shops. The assignment strengthened regulatory capacity for pharmacy outlet oversight.

Essentials of Pharmacy Practice Syllabus for Church Hospital Pharmacies

Apotheker developed a syllabus on Essentials of Pharmacy Practice for non-pharmaceutical staff working in church hospital pharmacies across Sub-Saharan Africa. The work supported safer medicine handling and stronger pharmacy practice in resource-constrained institutional settings.

Afya-Tek: digital health coordination from community
to facility.

Afya-Tek began as a digital-enabled health initiative implemented by Apotheker Consultancy from 2019 to 2022. It links Community Health Workers, private-sector drug dispensers, and health facilities into a digitized continuum of care for pregnant women, children, and adolescents.

The initiative demonstrates Apotheker’s ability to move from technical design into real-world implementation — combining community engagement, digital tools, referral coordination, training, data use, and public-private collaboration within Tanzania’s health system.

A rapid assessment explored the introduction of a group purchasing organization for medicines and medical supplies in Tanzania, contributing to stronger private-sector supply system thinking.

PointMed digital tool for ADDO business operations

The PointMed pilot introduced a point-of-sale and patient record software solution to support ADDO business management and improve record keeping at community medicine outlet level.

Amoxicillin-DT accessibility and availability evaluation

Through Results for Development, Apotheker supported evaluation work focused on improving access and availability of Amoxicillin-DT in ADDOs in the Dar es Salaam Region.

Improved Community Health Fund linkage with ADDOs

In collaboration with the University of Basel and support from the Swiss National Science Foundation, Apotheker assessed the feasibility and modalities of linking iCHF services with ADDOs.
